Book to join us for a webinar for Islington health, socialcare and VCS professionals - to find out about the Age UK Islington 'Planning for the Future' service (formerly Future Matters).

The Planning for the Future service provides free to access support for Islington residents / with an Islington GP - to get a Lasting Power of Attorney put in place, a Universal Care Plan (the new digital personalised care and support plan), Will or Funeral Plan organised.

Find out why it is so important to have Lasting Powers of Attorney in place for both health and welfare and for financial affairs, and how Age UK Islington is supporting Islington residents to put LPAs in place via the Planning for the Future service.

1.15pm - An introduction to Universal Care Plans

Joanie Speers, Age UK Islington

Find out the latest developments about Universal Care Plans, why they are important and how Age UK Islington is supporting Islington residents to put UCPs in place.

UCPs reduce duplication of data and can improve outcomes for patients with long-term conditions and those nearing the end of life. They enable greater efficiencies for the NHS - fewer unplanned hospital admissions and a higher likelihood of receiving care in their preferred location.
